Signed Board of Equalization Form 410-D is Required
California Department of Tax and Fee Administration (formerly Board of Equalization) Form 410-D is Required for All Vendors

Every vendor is required to completely fill out, sign and date the 410-D form.

No exceptions!

Forms will be provided by management (*or can be downloaded below) and then collected and mailed weekly to the CDTFA.

 

Please download, print and carefully read the Farmers Market Rules and Regulations before submitting an application to sell at The Farmers Market.

After you have read and understand the Rules and Regulations, print and complete the application which pertains to you.

Submit with any samples or photos, if necessary.  No incomplete applications will be accepted. 
 

CDTFA Form 410-D (*required for EVERYONE weekly)

Below applies ONLY to Farmers and Food Vendors, not Crafters.

Farmers must complete a load list and submit to the Market Manager before leaving the Market each week.

No exceptions.
